Please fill out the following form if you would like to become a patient of Rose Psychiatry. Please note that we do not accept any forms of insurance, but we can provide documentation so that you can file for reimbursement from your insurance provider. We respond to new patient requests during normal business hours Monday-Friday 9:00AM-5:00PM. We receive several new patient requests per day, so please be patient and our front office will get back to you as soon as possible. If you submit a new patient request on a Friday, please note that we may not be able to get in touch with you until the following work week. We appreciate your patience!
